What is the Pharmacy First service?
Pharmacy First is an NHS-funded service launched in January 2024 that lets community pharmacies treat seven common conditions directly — including supplying prescription-only medicines where appropriate — without you needing to see a GP first. The point is simple: take the load off general practice and get you treated faster for things a pharmacist is already qualified to handle.
You don't pay for the NHS pharmacy consultation itself. If you do need a medicine and you normally pay for prescriptions, the standard charge applies; if you're exempt, it's free.
The seven conditions Pharmacy First covers
The clinical pathways are tightly defined. We can offer a same-day consultation and, where the criteria fit, supply treatment for:
- Sinusitis — adults and children aged 12 and over
- Sore throat — aged 5 and over
- Earache (acute otitis media) — aged 1 to 17
- Infected insect bites — aged 1 and over
- Impetigo — aged 1 and over
- Shingles — aged 18 and over
- Uncomplicated urinary tract infections — women aged 16 to 64
Each pathway has age, symptom-duration, and red-flag rules. If you fall outside them — say, a UTI in a man, or a sore throat that's been going on for three weeks — we'll explain why we're sending you to the GP rather than treat in the wrong lane. That's not bureaucracy; it's the difference between a safe outcome and a missed diagnosis.
How to access Pharmacy First in Gloucester
There are three routes in, and all of them lead to the same consultation room at the back of Brookfield Pharmacy.
- Walk in. No appointment needed during opening hours. Tell the counter what's going on and we'll fit you in — usually within twenty minutes.
- GP referral. Many Gloucester surgeries now route Pharmacy First eligible problems straight to us. If your practice told you to "see your pharmacist," that's this service.
- NHS 111. If you call 111 with one of the seven conditions, they can refer you directly to a participating pharmacy.
You don't need to be registered with us — only with any GP in England. Bring a list of your current medicines if you have one; it speeds things up.
What happens in the consultation
A typical Pharmacy First consultation at Brookfield Pharmacy takes ten to twenty minutes. We use a private room, not the counter. You'll be asked about:
- the symptoms, when they started, and how they've changed
- your medical history and current medicines
- any allergies
- whether you're pregnant or breastfeeding (relevant for several treatments)
Depending on the condition, we may need to look in your ears, throat, at your skin, or carry out a urine dip. If the clinical criteria are met, we'll either supply an over-the-counter treatment with proper advice, or — increasingly often — issue a prescription on the spot. The whole record goes back to your GP automatically, so your notes stay up to date.
If we decide the problem isn't a Pharmacy First condition, we'll either refer you back to your GP with a clear handover, or send you straight to A&E if it warrants it. We don't gatekeep what we can't safely handle.
What this service isn't
It's worth being clear about the edges, because every week someone walks in expecting something Pharmacy First doesn't cover:
- Not for chest pain, breathing problems, or anything that feels like an emergency — that's 999 or A&E.
- Not a replacement for your GP relationship for chronic conditions like diabetes, asthma, or high blood pressure (though we can help with medication reviews separately).
- Not a route to a private prescription you weren't going to get on the NHS anyway. The pathways are NHS guidelines; we follow them.
- Not mental health support — though we'll signpost you to the right place if that's what you need.
For everything outside the seven conditions, Brookfield Pharmacy offers a wide range of services — including travel vaccinations, flu jabs, earwax microsuction, weight management, and emergency contraception — most of them without needing a GP letter first.
Why use Pharmacy First instead of waiting for a GP appointment
Three reasons, in our experience:
- Speed. Most Pharmacy First problems are time-sensitive. Shingles works better with antivirals started within 72 hours. A UTI gets worse fast. We see you the same day; the GP queue might not.
- Continuity. We're around the corner. If the antibiotics aren't working in 48 hours, you can walk back in — you don't have to start the queueing process again.
- It frees the GP for what only the GP can do. Every Pharmacy First consultation is a GP appointment freed up for someone who genuinely needs it. The system only works if people use it.
Frequently asked questions
Is the Pharmacy First consultation actually free?
Yes — the consultation itself is free regardless of your prescription-charge status. If we issue a prescription medicine and you normally pay the NHS prescription charge, that still applies. If you're exempt (under 16, over 60, certain medical conditions, valid HC2 certificate, etc.), the medicine is free too.
Do I need to be registered with a Gloucester GP?
You need to be registered with a GP in England — it doesn't have to be in Gloucester. We can see Hucclecote residents, Gloucester city patients, and anyone passing through who meets the clinical criteria.
Can children be seen under Pharmacy First?
Yes, for the age-eligible pathways: sore throat from 5, earache from 1 to 17, infected insect bites from 1, and impetigo from 1. A parent or guardian must attend with them.
